Также имеются сведения, что новая коронавирусная инфекция влияет на развитие антибиотикорезистентности. Цель. Оценить течение и исход послеоперационного периода у пациентов с инфекционным эндокардитом на фоне новой коронавирусной инфекции в зависимости от показателей антибиотикограмм. Материалы и методы. Проведен ретроспективный анализ историй болезни 10 пациентов (мужчин, в возрасте от 47 до 70 лет) с инфекционным эндокардитом, находившихся на лечении в Федеральном государственном бюджетном учреждении «Федеральный центр сердечно-сосудистой хирургии» (г. Астрахань) с 2020 по 2023 г. Пациентов по наличию новой коронавирусной инфекции разделили 2-е группы: 1 группа (n = 5) – пациенты с инфекционным эндокардитом и новой коронавирусной инфекцией; 2 группа (n = 5) – пациенты с инфекционным эндокардитом без новой коронавирусной инфекции. Диагностику инфекционного эндокардита при поступлении в стационар и оценку противомикробной терапии осуществляли с помощью микробиологических исследований в соответствии с требованиями законодательства Российской Федерации. Пациенты в группах были сопоставимы по полу, возрасту и тяжести сердечно-сосудистой патологии. Результаты. У пациентов 1-й группы резистентность к противомикробным препаратам при поступлении в стационар в среднем составила 97,33 %, у пациентов 2-й – 40,81 %, что на 56,52 % ниже, чем в 1-й группе. Это указывало на роль новой коронавирусной инфекции в развитии резистентности у пациентов с инфекционным эндокардитом. У 1-й группы с множественной резистентностью и резистентностью к препаратам выбора исход был неблагоприятный. Выполненное исследование позволило выявить у пациентов с инфекционным эндокардитом и новой коронавирусной инфекцией множественную резистентность бактериальной гемокультуры к антибиотикам, связанную с тяжелым течением, что дает основание считать ее фактором риска неблагоприятного исхода заболевания. Поскольку лечение больных с множественной резистентностью проблематично, можно рекомендовать результаты анализа накопленных наблюдений для совершенствования существующих клинических рекомендаций по лечению инфекционного эндокардита у пациентов с новой коронавирусной инфекцией.</p></abstract><trans-abstract xml:lang="en"><p>There is strong evidence in the literature on the effect of multiple antimicrobial resistances on the course of the disease. There is also evidence that coronavirus disease 2019 affects the development of antibiotic resistance. The aim. To evaluate the course and outcome of the postoperative period in patients with infectious endocarditis against the background of coronavirus disease 2019, depending on the indicators of antibiotics. Materials and methods. A retrospective analysis of the case histories of 10 patients (men, aged 47 to 70 years) with infective endocarditis, who were treated at the Federal State Budgetary Institution Federal Center for Cardiovascular Surgery (Astrakhan) from 2020 to 2023, was carried out. Patients in the presence of coronavirus disease 2019 were divided into 2 groups: group 1 (n = 5) – patients with infectious endocarditis and coronavirus disease 2019; group 2 (n = 5) - patients with infectious endocarditis without coronavirus disease 2019. Diagnosis of infective endocarditis upon admission to the hospital and assessment of antimicrobial therapy was carried out using microbiological studies in accordance with the requirements of the legislation of the Russian Federation. Patients in the groups were comparable in gender, age and severity of cardiovascular pathology. The results. In patients of the 1st group, antimicrobial resistance at admission to the hospital averaged 97.33 %, in patients of the 2nd – 40.81 %, which is 56.52 % lower than in the 1st group. This indicated the role of coronavirus disease 2019 in the development of resistance in patients with infectious endocarditis. Group 1 with multiple and choice drug resistance had an unfavorable outcome. The performed study revealed multiple antibiotic resistance of bacterial hemoculture in patients with infectious endocarditis and coronavirus disease 2019 associated with a severe course, which gives reason to consider it a risk factor for an unfavorable outcome of the disease.
