В условиях отсутствия унифицированных национальных рекомендаций по срокам и частоте мониторинга особую клиническую значимость приобретает комплекс диагностических методик, направленный на выявление доклинических форм миокардиальной дисфункции. К числу наиболее информативных методов относятся: трансторакальная эхокардиография с оценкой глобальной продольной деформации методом speckle-tracking, а также определение кардиоспецифических биомаркеров, включая тропонины T и I и натрийуретические пептиды. Применение указанного комплекса диагностических исследований позволяет осуществлять раннюю верификацию кардиотоксических эффектов, что имеет принципиальное значение для своевременной коррекции терапии.</p></abstract><trans-abstract xml:lang="en"><p>This review presents current laboratory and instrumental methods for cardiovascular toxicity diagnosing in patients with multiple myeloma undergoing autologous hematopoietic stem cell transplantation. In the absence of national guidelines on the timing and frequency of monitoring, it is particularly important to have a set of techniques to detect preclinical stages of myocardial dysfunction. The most useful methods include transthoracic echocardiography, with assessment of global longitudinal strain using speckle tracking, and measurement of cardiac biomarkers such as troponin T and I, and natriuretic peptides.
