Цель: проанализировать частоту и сроки развития гипокоагуляционных и тромбоэмболических осложнений в зависимости от приверженности пациентов к лечению после протезирования митрального клапана механическими протезами через 6, 12 и 60 месяцев. Материалы и методы. Обследовано 260 пациентов, прооперированных по поводу протезирования митрального клапана механическими двустворчатыми протезами. Для оценки комплаентности использовался тест Мориски-Грина (4-item Morisky Medication Adherence Scale - MMAS-4). Результаты исследования. У пациентов после протезирования митрального клапана механическими протезами частота развития тромбоэмболических и гипокоагуляционных осложнений была выше у некомплаентных пациентов на всех этапах исследования. Статистически значимые различия в частоте развития тромбоэмболических осложнений были выявлены на этапе 6 месяцев, гипокоагуляционных - на этапе 60 месяцев наблюдения. При этом шансы развития тромбоэмболических осложнений на этапе 6 месяцев наблюдения у комплаентных пациентов были в 3,4 раза ниже по сравнению с некомплаентными пациентами. А шансы развития гипокоагуляционных осложнений на этапе 60 месяцев наблюдения уменьшались в 1,2 раза у комплаентных пациентов. Заключение. Таким образом, одним из направлений для уменьшения риска развития тромбоэмболических и гипокоагуляционных осложнений у пациентов после протезирования митрального клапана механическими протезами является повышение комплаентности пациентов.
The aim of the study. To analyze the frequency and timing of the development of hypocoagulation and thromboembolic complications, depending on the adherence of patients to treatment after mitral valve replacement with mechanical prostheses after 6, 12 and 60 months. Materials and methods. We examined 260 patients operated on for mitral valve replacement with mechanical bivalve prostheses. To assess compliance, the Morisky-Green test (4-item Morisky Medication Adherence Scale - MMAS-4) was used. Research results. In patients after mitral valve replacement with mechanical prostheses, the incidence of thromboembolic and hypocoagulation complications was higher in non-compliant patients at all stages of the study. Statistically significant differences in the incidence of thromboembolic complications were revealed at the 6-month follow-up stage, in the incidence of hypocoagulation complications at the 60-month follow-up stage. At the same time, the chances of developing thromboembolic complications at the stage of 6 months of follow-up in compliant patients were 3.4 times lower compared to non-compliant patients. And the chances of developing hypocoagulable complications at the stage of 60 months of observation decreased by 1.2 times in compliant patients. Conclusion. Thus, one of the ways to reduce the risk of thromboembolic and hypocoagulant complications in patients after MV prosthetics with mechanical prostheses is to increase patient compliance.
